General Services List

The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected.

SERVICES, FACILITIES, AND MOTOR EQUIPMENT

Transfer of remains to funeral home (within 30 miles)

Basic services of funeral director and staff Our fee for the basic services of funeral director and staff include conducting the arrangement conference, planning the funeral, obtaining necessary permits, shelter of remains and placing obituary notices. This fee for our basic services will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, forwarding or receiving remains and anatomical gifts.) Embalming Except in certain special cases, law does not require embalming. Embalming may be necessary, however, if you select certain funeral arrangements such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it such as direct cremation or immediate burial.

ALTERNATIVE SELECTIONS Forwarding of remains to another funeral home Our charge for this service includes transfer of remains to funeral home, embalming, casketing, dressing and cosmetizing, basic services of funeral director and staff, motor equipment to point of transportation and service and utility vehicle. This charge does not include viewing/visitation, ceremony, and merchandise, shipping charges or transportation outside a 30-mile radius.

Receiving remains from another funeral home. Our charge for this service includes, basic services of funeral director and staff, transfer of remains to cemetery or crematory and service and utility vehicle. This charge does not include viewing/visitation, ceremony, graveside service, merchandise, and transportation outside a 30-mile radius or cemetery or crematory fee.

Direct cremation available from This charge includes removal of remains, service and utility vehicle, transfer to crematory, crematory fee, transfer of remains to funeral home, other preparation of body, refrigeration and basic services of staff. This charge does not include any rites or ceremonies, or urn. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are made of fiberboard. Immediate burial. This charge includes removal of remains, local transportation to the cemetery, washing and disinfecting, casketing, dressing and cosmetizing remains, basic services of the staff or service and utility vehicle. This charge does not include any rites or ceremonies or cemetery fee. This service includes use of stateroom for guest registration for one or more days prior to service time.

Traditional Chapel/Church Service, no viewing/visitation/stateroom

Graveside Service, with viewing/visitation/stateroom
This service includes: Transfer of remains to funeral home, Services of Director and Staff, Embalming, Other Preparation of the body, Use of Facilities and staff for viewing/visitation/stateroom, Staff for Graveside Service, Hearse, Family Car and Service Car.

Option 4B

Graveside Service, no viewing/visitation/stateroom
This service includes: Transfer of remains to funeral home, Services of Director and Staff, Embalming, Other Preparation of the body, private family viewing for one hour, Staff for Graveside Service, Hearse, Family Car and Service and Utility Vehicle.

Option 5B
Basic Immediate Burial, with no attendant services
This service includes: Transfer of remains to funeral home, services of director and staff, Hearse, Service and Utility Vehicle, private family viewing for one hour, other preparation of body, refrigeration, casketing, and dressing deceased.
The above service packages do not include merchandise, such as casket, alternative container, vault, vault service charge, urn, register book, memorial folders, prayer cards, acknowledgment cards, crucifix, additional laminations, or cash advance items and sales tax. Cremation packages include the crematory fee.

Use of facilities for memorial service, Private family viewing for one hour, Committal service, Family Car, Service and Utility Vehicle, Crematory Fee and Transfer to Crematory

Graveside Memorial Cremation Service (no viewing/visitation)
This service includes: Transfer of remains to funeral home, Services of Director and Staff, refrigeration and other preparation of body. Use of Staff for a graveside memorial service, private family viewing for one hour, Family Car, Service and Utility Vehicle, crematory fee and transfer to crematory.

Basic Direct Cremation, with no attendant services
This service includes: Transfer of remains to funeral home, services of director and staff, crematory fee, refrigeration, other preparation of body, private family viewing for one hour, service and utility vehicle and transfer to crematory.
The above service packages do not include merchandise, such as casket, vault, alternative container, vault service charge, cremation urn, register book, memorial folders, prayer cards, acknowledgment cards, crucifix, additional laminations, or cash advance items and sales tax. SERVICES FOR A STILLBORN, INFANT OR CHILD

Death always is accompanied by a feeling of loss. At the Bath-Naylor Funeral Homes, Pittsburg, Weir & Columbus and the Friskel Funeral Home in Frontenac, we feel a special sense of loss in the death of a youth. Therefore, we have adopted the following policies regarding services to families in our area.


Age 5 and Under
There will be no charge for services of our staff and the customary use of our facilities. In addition, we will provide our basic infant casket without charge. Alternative caskets are available at our listed retail prices, and such costs as cemetery/crematory charges, florists, clergy honorariums, musicians and transportation outside a 30 mile radius, are the responsibility of the family.
Age 6 through 10
There will be no charge for our standard service fee for the services of our staff and customary use of our facilities. Caskets and other merchandise are available at our listed retail prices and such costs as cemetery/crematory charges, florists, clergy honorariums, musicians and transportation outside a 30 mile radius, are the responsibility of the family.
Age 11 through 16
We will reduce our standard service fee for the services of our staff and the customary use of our facilities by 50%. Caskets and other merchandise are available at our listed retail prices and such costs as cemetery/crematory charges, florists, clergy honorariums, musicians and transportation outside a 30 mile radius, are the responsibility of the family.
SRS ALTERNATIVE BURIAL AND CREMATION SERVICES

SRS ALTERNATIVE FUNERAL SERVICE
This service includes the following: Transfer of remains to funeral home (within 30 miles), Basic services of funeral director and staff, Embalming, Casketing and dressing, Use of facilities for viewing/visitation, Use of facilities for funeral ceremony or church service and use of staff for a graveside service, Hearse, Family Car, Service Car (within 30 miles), and a cloth covered casket.

SRS ALTERNATIVE MEMORIAL CREMATION SERVICE
This service includes the following: Transfer of remains to funeral home and crematory, service car, (within 30 miles), basic services of funeral director and staff: washing and disinfecting the remains; refrigeration, use of facilities for memorial service; crematory fee; temporary cremation urn; and an alternative cremation container, private family viewing for one hour.


SRS ALTERNATIVE DIRECT CREMATION SERVICE
This service includes the following: Transfer of remains to funeral home and crematory, service car (within 30 miles); basic services of funeral director and staff; washing and disinfecting the remains; refrigeration, private family viewing for one hour, crematory fee; temporary cremation urn; and an alternative container.

The services listed above carry a discount over our normal service charge and are offered as a public service to families of our area that qualify for SRS funeral assistance. Payment is due in full on or before the day of the service for all charges incurred. This service is not subject to any other discounts or substitutions of merchandise.
